Supervision and assistance are delivered on an individual or group basis, in the home or community

Group and individual transport assistance to connect an older person with their usual activities. Direct (driver provided) and indirect (taxi/rideshare vouchers)
Assistive Technology (AT)
Devices or equipment that help with daily living or mobility, such as:
- Mobility aids
- Personal alarms or medical alert systems
- Hearing or vision aids
- Medication management tools
- Smart home devices
Home Modifications
Changes to the home environment to improve safety and accessibility:
- Grab rails and handrails – in bathrooms, hallways, or near stairs
- Ramp installation
- Non-slip flooring or bathroom modifications
- Widening doorways – to accommodate mobility aids
- Improved lighting
